IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Availability of 2025 Tax Year IRS Forms 1095-B and 1095-C

In accordance with the Affordable Care Act and the Paperwork Reduction Act provisions, Cass County will provide Forms 1095-B and 1095-C to employees upon request, rather than automatically distributing them. These forms provide information about the offers of coverage made to employees to enroll in the county’s health plans. This also includes former employees who are enrolled in Cass County’s MERS HCSP (Healthcare Savings Plan). Employees and former employees may request a copy of their 2025 Form 1095-B or 1095-C at no cost.
